Encounter

A guided selector records systems, organs and common examination findings with explicit source and assessment. Full-history, SOAP and SBAR formats reorganize recorded text. Core fields are international; local context fields are optional. Sections cover the presenting concern, personal and family history, medications, allergies, social context, review of systems, vital signs, attributed examination findings, investigations with units and source, assessment, differential diagnosis, plan and progress. Pediatric and obstetric profiles add relevant history.

Temporary saving, privacy and scope

The draft lasts two hours in this browser tab, unless you explicitly extend it before expiry. Closing the tab usually clears it; browser session restoration can preserve it until expiry. Other tabs and devices do not share the draft. Undo/redo retains the last 20 changes only in memory during this session and resets on reload or a new case. On a shared device, clear the session before leaving. Browser storage is not a secure clinical-record system.

Downloaded PDFs persist independently. The tool does not diagnose, prescribe, grade or record audio/video. Do not paste case details into the Global search or chat.
