Before a minor can book routine care, the clinical team must review a signed parent or legal guardian authorization, verify the signer’s national ID and legal authority, and confirm suitable care.

Prepare with the clinician
Read the service, privacy and crisis information. Ask questions about the professional, approach, fees, confidentiality and alternatives. The blank form must be completed with the assigned care team.

Verify representation privately
The signer must supply a national ID through the private clinical channel. Identity alone does not establish guardianship: the team reviews parental authority, custody restrictions or supporting documents when needed.

Listen to the young person
The clinician explains care in an age-appropriate way, considers understanding and assent or dissent, and explains what remains confidential and when safeguarding requires disclosure.

Receive documented approval
The team checks authority, consent, suitability, location and the emergency plan before routine booking. A signature or website checkbox alone is not approval.

The provider must identify the record custodian, purpose, authorized access, retention, rights-request procedure and any lawful transfers before receiving documents. Only necessary information is collected. Recording, research, publicity, student observation and AI training need separate permission; refusing optional uses does not block ordinary care.
